Has your dog ever

- been involved in an accident or suffered serious injury?

- slipped or fallen when running around or playing?

- been thrown forwards or backwards while traveling in a car or truck?

Does your dog

- work regularly as a beating or pick-up dog (or similar)?

- pull heavily on the lead when out for a walk?

- play roughly in ball games, with tug toys or with other dogs?

- run up and down stairs or on slippery surfaces without care?

- participate regularly in agility competitions or other disciplines?

- show stiffness getting out of bed or after exercise?

- seem uncomfortable when moving around?

- show obvious lameness whether constant or intermittent?

- have difficulty getting up or down stairs or into or out of a car?

- tiring easily for no obvious reason?

- reluctance to work, play, lie or sit down?

- growling when a specific part of the body is touched?

- reluctance to play or chase balls?

- reluctance to sit, lie down, or perform agility or obedience tasks?

- carrying the head, back-end or tail to one side?

- lying down to sleep on one side only?



Has your horse ever

- fallen or suffered serious injury?

- had saddle-fitting or rug fitting problems?

- compete regularly in competitions?

- have stiffness after rest or exercise?

- take longer than usual to warm up when schooling or competing?

- experience lameness whether constant or intermittent?

- show ‘hanging’ or stiffness on one rein/side when schooling?

- have difficulty striking off into canter?

- throw up its head during transitions between paces?

- refuse to jump or jump consistently to one side?
